Home Care is healthcare or supportive care provided in the patient's home by licensed healthcare professionals. Home Care may also be referred to as social care or domiciliary care. The licensed personnel and others who assist the individual are often referred to as caregivers who may help the individual with such daily tasks as bathing, eating, cleaning the home and preparing meals. For increased needs, Home Care may also include hospice care and rehabilitative assistance.
The interRAI Community Health Assessment tool (interRAI CHA) is a comprehensive assessment system for the Home Care market. Every patient receives the core assessment called the CHA and only those with advanced needs receive one or more of the additional four supplements. This modularized approach to assessment facilitates tracking of a patient’s needs using the fewest number of assessments.
