MDS-HC supports single point of entry for home care providers and other community service organizations through the continuum of care cycle. This module automatically generates a care plan for a client once the MDS-HC assessment is completed. This care plan can then follow the client when admitted to a facility. If the facility has MED e-care's iPlan software the data collected using MDS-HC can be electronically transferred to that facility.
